| OrderingOrdering is simple. Here is the procedure: 1. Select an item number from any of the following sections: Plates, Trays, Bowls, Vases, Votives, Lamps. 2. Select a pattern number from any of these pattern sections: Flowers, Fruits and foods, Doily, Holiday, Nautical, Bird, Judaica, or Other. 3. If you would like a Hebrew saying on your piece, select a Hebrew Saying number in the Judaica pattern section. 4. Then print out the order form, fill it in with the appropriate information and send it to Distinguished Art Glass with your check or charge card number. Charge card items will be shipped right away, if in stock. Orders for which you pay by check will be sent after the check clears. Allow 4-6 weeks for us to create items that are not in stock.

But in this case, it would help to fill out your order form first to simplify ordering. Please do not include your charge card number in an email. Note that if there are any problems with your order (for example your choice of a pattern and a Hebrew saying are not compatible), Distinguished Art Glass will contact you for clarification. Shipping and Handling Costs: Glass needs to be packaged very well and handled carefully. Shipping and handling adds 15% to your total cost. This includes UPS costs, boxes, bubble wrap, everything.
ExampleSuppose you would like a large section plate (Item # PL2) with the Mushroom pattern (FR1_7), but no Hebrew saying. Your order form would look like this:
